1) Go to Appearance >> Menu >> Create new menu name and add pages.
2) Go to Appearance >> Widgets >> Footer column 3 >> Add custom menu widget and select the menu you have created from the dropdown.

You need to access the header.php file by go to Appearance >> Editor >> header.php and add the code between <head> tag.
